File A Claim

If you are a Class Member and wish to be potentially eligible for a payment from the proceeds of the Settlement, you must complete and submit a Proof of Claim and Release ("Proof of Claim"). To obtain a copy of the Proof of Claim, please click here. The deadline to submit a timely Proof of Claim is April 15, 2019.

You may submit your Proof of Claim using one of two methods. Each method is described below, with links to the requirements for each submission. Your submitted Proof of Claim will include a release of your legal claims against each and all of the Released Defendant Parties (as defined in the Stipulation of Settlement), as well as a certification under penalty of perjury that your Proof of Claim is accurate and complete.

Whichever submission method you choose, please note that supporting documentation of the transactions and holdings listed on your Proof of Claim is required. Include all required and necessary documentation with your submission. In the event that you submit your Proof of Claim online, you must retain confirmation of such filing, which is provided at the end of the filing process.

If you fail to execute and submit a timely, properly completed and addressed Proof of Claim, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.

Documentation

We do not hold stock records. Gilardi & Company, LLC is a third-party administrator appointed by the Court to notify potential claimants. We are provided with names and addresses of potential claimants in order to notify individuals of settlements but do not have access to your private investment records. We have provided some direction below on how you may potentially obtain the required documentation to support your claim submission.

ASPP, DSPP, Employee Stock Options, Asda (UK Walmart Affiliate)

If, during the Class Period, you purchased or acquired Walmart publicly traded common stock shares through Walmart’s Associate Stock Purchase Plan (ASPP) or Direct Stock Purchase Plan (DSPP), had employee stock options which were exercised during the Class Period for Walmart publicly traded common stock shares (stock options themselves are not eligible for inclusion) and/or elected to receive Walmart publicly traded common stock shares from your Asda UK ShareSave Plan during the Class Period, you can contact Walmart Shareholder Services via toll-free # (800) 438-6278 for the necessary documentation to support your claim submission. They can also be reached via email at Walmart@computershare.com and the website is www.computershare.com/walmart.

401(k) Plan

In some cases the plan trustee or fiduciary may choose to submit a claim on behalf of all plan participants. We cannot advise at this time if the Walmart 401(k) plan is doing so in this matter. If, during the Class Period, you were an associate/employee of Walmart and purchased or acquired Walmart publicly traded common stock shares through Walmart’s 401(k) plan, you should contact your plan/benefits administrator or to determine if they will be submitting a claim on behalf of the plan and its participants or request documents to submit your own claim, if the plan will not be submitting. However, we do not have contact information for participants to contact the plan/benefits administrator.

If a claim is submitted by the 401(k) plan on behalf of all participants, that claim would supersede any individual claim which you have submitted and you would be notified that your claim has been rejected as a duplicative submission, and any distribution would be issued to the 401(k) plan directly.

Individual Brokerage/Investment Account

We recommend you contact the broker/institution where you held your shares and request proof or supporting documentation for your claim.

If you are unsure of where you held your shares, please send an email to info@WalmartSecuritiesSettlement.com with the name of the case and the name and mailing address to which notice was sent. In your email, please ask us to research and see if we can identify the name of the institution which provided your name and address. Please keep in mind, however, that we send out unaddressed packages of blank claim forms to brokers, transfer agents, etc. when requested, and those claim packets may be mailed directly by the financial institution to their clients. As such we are not always able to identify which institution may have record of you as a potential class member.

Submit Your Proof of Claim by Mail

To submit a Proof of Claim by mail, complete and sign the Proof of Claim in its entirety with all required supporting documentation and mail it, postmarked no later than April 15, 2019, to the Claims Administrator at:
